The Story

Three Months Ago, My Wife Could Barely Walk

I am going to tell you a story I have never shared publicly before — because it is not about a patient. It is about my wife.

Three months ago, she could barely walk. She could not pick up our five-year-old son. She could not carry groceries. The Real Problem

Why Adjustments, Stretching, and Core Exercises Have Not Fixed Your Back

There is nothing wrong with chiropractic adjustments. There is nothing wrong with stretching. There is nothing wrong with core exercises. The problem is when any of those things are used in isolation — without understanding what is actually driving your pain, without objective testing, and without a progressive plan that builds real capacity over time.

Here is what the research actually shows. A 2023 clinical practice guideline in the Journal of Orthopaedic & Sports Physical Therapy found that for chronic low back pain, supervised progressive exercise combined with manual therapy produces significantly better outcomes than either approach alone.[1] A 2021 Lancet systematic review confirmed that exercise therapy is the single most effective non-pharmacological intervention for chronic low back pain — but only when it is individualized, supervised, and progressively loaded.[2]

Generic stretching sheets and "strengthen your core" advice fail because they skip the most important step: zooming out and assessing the entire body as a connected system. Your low back pain might be driven by weak glutes that cannot stabilize your pelvis when you walk, carry your child, or climb stairs. It might be driven by a stiff thoracic spine that forces your lumbar spine to compensate with excessive motion at every turn. It might be a motor control problem — your core is not firing when it should, so other structures are picking up the slack. It might be a hip that has lost so much strength, mobility, and symmetry side to side that your back is absorbing forces it was never designed to handle alone. It might be a disc that needs directional loading, not generic stretching.

Until someone assesses all of that — the mobility, the motor control, the strength, the symmetry, the way your body actually moves under load — through objective testing, not guessing — every treatment is a shot in the dark. And shots in the dark produce inconsistent results. That is not a controversial statement. That is the clinical reality.

The gap nobody talks about: pregnancy and postpartum recovery fundamentally change how a woman's body distributes load. Is low back pain after pregnancy something I just have to live with?

No. Pregnancy changes how your body distributes load — the deep core, pelvic floor, hip stabilizers, and lumbar spine all adapt. But those compensations can be retrained. The problem is that most women never get a progressive rehabilitation plan after pregnancy. They get adjustments or stretches, and when the relief does not last, they assume the pain is permanent. I have been told to strengthen my core. Why has that not worked?
